About the Service 

The Community Cancer Health and Wellbeing Support Service provides personalised, holistic support for individuals living with and beyond cancer, as well as their families and carers.
Delivered by the Wellbeing Network, this service is embedded within a network of local community hubs across Bradford and Craven, ensuring accessible, community-based care. 

 

What We Offer 
  • Emotional wellbeing and referral to counselling support 
  • Financial and welfare advice 
  • Access to local community groups and voluntary sector services 

 

Eligibility 

You can access this service if you are: 

  • An adult aged 18 or over 
  • Living in Bradford District or Craven 
  • Diagnosed with cancer (any stage) 
  • A family member or carer of someone with cancer 

 

Referral and Access 

The service is accessible via a Single Point of Access (SAP) for both professionals and individuals.
Referrals can be made by GPs, hospital teams, community health professionals, or through self-referral. 

Walk-in access is also available at our community hubs.
