About the Falls Prevention Programme
The wider Falls Prevention Programme in Bradford District and Craven includes two complementary community elements:
Falls Champions – developing the knowledge, confidence and capacity of VCSE organisations to contribute to falls prevention, strengthening community pathways and improving connections with Primary Care and clinical services.
Walk From Home – a separate supported scheme designed to help people regain confidence with small everyday activities and increase their independence following a fall, deterioration in mobility or clinical intervention.
Falls Champions are therefore part of a wider system of prevention rather than a standalone service.
Initially, the wider programme will focus particularly on people aged 65 and over who are at intermediate to high risk of falling.
Falls Champions EOI
The Wellbeing Network is seeking delivery partners interested in delivering the work in Craven & Wharfedale or Keighley & Airedale to host and deliver the Falls Champion element of the new Proactive Falls Prevention programme.
The programme is a partnership initiative bringing together the VCSE sector, Primary Care, Community Therapy and the BDCFT Falls Prevention Team. Its purpose is to strengthen low-level, preventative falls support within communities, improve connections between health services and community provision, and help people remain active, confident, and independent.
The Falls Champion element will operate from September 2026 to March 2028.
Providers are invited to express an interest in hosting a 0.6 FTE Falls Champion for an agreed geographical locality. The funding available is £43,707 per locality for the work.
What are Falls Champions?
Falls Champions will build the capacity of community organisations to help prevent falls, and support people to remain safe and independent. They will deliver falls prevention awareness sessions, work with Primary Care, Falls Prevention and Wellbeing Network partners, map local activities, and strengthen referral and signposting routes.
They will also work alongside the Walk From Home scheme (more information below), helping people move from short-term confidence-building support into longer-term, sustainable community activities.
Walk From Home Volunteer Coordinator EOI
The Wellbeing Network is seeking an organisation to take responsibility for the district-wide coordination of the Walk From Home volunteering offer across Bradford District and Craven, as part of the wider Pro-active Falls Prevention programme.
The provider will establish and coordinate a safe, consistent and accessible volunteer offer, including recruitment, training, support, referral coordination, matching, monitoring and partnership working.
Crucially, this is a partnership coordination role delivered through a phased implementation approach. The successful provider will be expected to work across organisational boundaries, build capacity over time, and draw on the strengths of the wider Wellbeing Network and VCSE sector rather than developing an isolated volunteering service.
The provider will work particularly closely with the five locality Falls Champions. While the Champions build falls prevention capacity, relationships and community pathways within their localities, Walk From Home will provide direct, practical volunteer support to people who need help rebuilding confidence and independence.
The total funding for this work is £62,417 for the delivery period September 2026 to March 2028.
What is the Walk From Home Scheme?
The Walk From Home scheme will provide trained volunteer support to help people rebuild confidence, mobility and independence through small, achievable everyday activities. It will deliver low-level preventative intervention. Examples of this include supporting someone to: walk to their local shop, put their bins out, take a short local walk, and more.
The intention is not to replace clinical rehabilitation, personal care or specialist falls interventions. Instead, Walk From Home provides a bridge between someone’s home, their recovery and their wider community, using appropriately trained and supported volunteers.
Mobilisation and delivery will be phased across Bradford District and Craven, allowing the scheme to build volunteer capacity, develop local partnerships and establish effective referral pathways as it rolls out across different areas.
